Part 21 - 2003-0031-00 - Initial/Interim Report on Fuel Assembly Bow

FANP is reevaluating their fuel assembly bow analyses for Mark-B and Mark-BW fuel. The ongoing evaluation of the methodology will include additional investigations on the gap size assumed in the analysis. In addition, the entire analytical approach, including the assumptions used, will be evaluated as a whole to determine whether any changes in the methodology are appropriate to ensure adequate safety. No conclusions have been reached by Framatome ANP regarding reportability under Part 21 at this time. The scheduled evaluation completion date is 12/17/2004. Discussion:

Part 21 - 2003-0028-00 - Solid State Protection Master Relay

This Morning Report is being issued due to a safety concern regarding the failure of Master Relay K507 (MidTex Part Number 156-14D200) in the Solid State Protection System (SSPS) at Summer. This failure occurred on 10/03/03 and was reported on 12/01/03 in EN 40364. The failures were attributed to corrosion caused by off-gassing of chlorine contamination from paper used to wrap the relay coil, or the adhesive used to attach the paper wrap to the coil. Another MidTex P/N 156-14D200 relay with a paper wrapped coil was sent to Pentas for followup analysis, and it showed evidence of the same type of corrosion. Therefore, the failure of Master Relay K507 is considered to be a manufacturing defect in a basic component that could cause a loss of safety function.

According to the licensee, the majority of MidTex relays investigated had cloth wrapped coils and these relays showed no evidence of the corrosion found on paper wrapped coils.

Upon energization, master relays must change state to actuate various safety related equipment in the plant. The number of affected relays and installed locations was not available at the time of the initial notification. The NRC staff has requested additional information from the licensee pertaining to this issue and is awaiting a reply. As the licensee provides more information, it will be posted to our webpage.

Discussion:

On January 13, 2004, Mano Nazar, Senior Vice President and Chief Nuclear Officer, announced that Robert Gillespie will succeed Susan Simpson as the D.C. Cook Plant Operations Director. Mr. Gillespie is expected to assume his new position within the next 2weeks. Mr. Gillespie will join AEP from the Nuclear Management Company, where he was recently the Operations Manager at the Prairie Island Nuclear Plant. Ms. Simpson will become the Learning Organization Department Manager effective February 1, 2004.

These management changes were announced to better align the licensee's organization in support of its Recovery Plan to improve plant performance.
